Dance Partners must complete this form after each participatory dance event. Please be as detailed as possible, thank you.

    Provide a brief description of how the event went.

    Including:
    • What went well about the event
    • Were there any challenges before, during or after the event?

    Action Items from Event Report and Notes

    This question should be filled out for issues such as:
    • If Park Enforcement needed to be called
    • Participant feedback
    • If a staff member or participant had a medical event (passed out, overheated, etc.)
